PERSONAL DATA PROTECTION POLICY

1.  INTRODUCTION

This Personal Data Protection Policy Statement sets out how Well-Wellness Crown Pte. Ltd. (Well-Wellness Crown) uses and protects any personal information that you have consented to provide in accordance with the Personal Data Protection Act of Singapore 2012. This document contains information about how and why we collect, use and disclose your personal data and the assurance that it will only be used in accordance with this Policy Statement.

2.  YOUR PERSONAL DATA

2.1 In this Personal Data Protection Policy, "Personal Data" refers to any data or information about you from which you can be identified either (a) from that data; (b) from that data and other information to which we have or are likely to have access. Examples of such Personal Data which you may provide to us include but are not limited to

1. your name, NRIC, passport or other identification number, telephone number(s), mailing address, email address and any other information relating to you which you have provided us in any forms you may submitted to us, or in other forms of interaction with you;

2. information about your use of Well-Wellness Crown website and services, including cookies, IP address, subscription account details and membership details, but only to the extent that Well-Wellness Crown may identify you from such information;

3. your employment history, education background, and income levels;

4. your transaction related information, such as your bank account or credit card information, and your credit history; and

5. information about your usage of and interaction with our website and/or services including computer and connection information, device capability, bandwidth, statistics on page views, and traffic to and from our website.

3.  COLLECTION OF PERSONAL DATA

3.1 Your personal data may be collected in several ways, including but not limited to the following:

1. when you register for a membership with us;

2. when you submit any other application form(s) relating to our products and services;

3. when you submit an application for employment;

4. when you access our website and perform an online transaction;

5. when you interact with any of our employees (including customer service officers);

6. when you make a purchase from us;

7. when you respond to our request for additional Personal Data;

8. when you ask to be included in an email or other mailing list;

9. when you respond to our initiatives or promotions; and

10. when you submit your Personal Data to us for any other reason.

4.  PURPOSE OF COLLECTION, USE AND DISCLOSURE OF YOUR PERSONAL DATA

4.1 Well-Wellness Crown may generally collect, use and/or disclose your Personal data for the following purposes: 

1. evaluating and providing advice and/or recommendations to you regarding the type of products and services suited to your needs;

2. responding to your queries and requests and handling of complaints;

3. conducting and organising company activities which includes services, courses, activities and events;

4. administering, maintaining, managing and operating the products and services offered to you, including our relationship with you;

5. internal and external communication and publication;

6. delivering information regarding news, publications and events;

7. internal record keeping;

8. conducting market research for statistical profiling and other purposes to understand and determine customer preferences and demographics in order for us to review, develop and improve the products and services which we are providing to you (including without limitation to ensure that the products and services offered are relevant to you);

9. the purposes for which we have specifically obtained your consent;

10. the purposes for which you have provided your Personal Data to us; and

11. any other purpose which are reasonably related to the aforesaid.

5.  DISCLOSURE OF PERSONAL DATA

5.1 Well-Wellness Crown will take reasonable steps to protect your personal data against unauthorised disclosure, subject to the provisions of any applicable law, relevant portions of the collected data may be disclosed, for the purposes listed above to the following:

1. our staff and members;
2. agents, contractors or third party service providers who provide services, such as telecommunications, mailing, information technology, payment, payroll, data processing, training, market research, newspaper vendor services, storage and archival or other services to Well-Wellness Crown
3. our professional advisors such as our auditors and lawyers;
4. banks, financial institutions, credit card companies and their respective service providers;
5. third party reward, loyalty, privileges and co-branding programme providers;
6. relevant government regulators, statutory boards or authorities or law enforcement agencies to comply with any laws, rules, guidelines and regulations or schemes imposed by any government authority;
7. any other party to whom you authorise us to disclose your personal data.

6.  USE OF COOKIES

6.1 We may collect or analyse anonymised information from which individuals cannot be identified ("Aggregate Information"), such as number of users and their frequency of use, the number of page views (or page impressions) that occur on our websites and common entry and exit points into our website.

6.2 We use "cookies" where a small data file is sent to your browser to store and track Aggregate Information about you when you enter our website. The cookie is used to track information such as the number of users and their frequency of use, profiles of users and their preferred sites.

6.3 The Aggregate Information collected is used to assists use in analysing the usage of our website and improving our website.

6.4 Should you wish to disable the cookies associated with these technologies, you may do so by changing the setting on your browser. However, you may not be able to enter certain part(s) of our website.

7.  THIRD-PARTY SITES

7.1 Our website may contain links to other websites operated by third parties independent of the Well-Wellness Crown. We are not responsible for the privacy practices of websites operated by third parties that are linked to our website. We encourage you to learn about the privacy policies of such third party websites. Some of these third party websites may be co-branded with our logo or trademark, even though they are not operated or maintained by us. Once you have left our website, you should check the applicable terms, conditions and policies of the third party website to determine how they will handle any information they collect from you.

8.  PROTECTION OF YOUR PERSONAL DATA

8.1 All personal data in our possession or control are kept confidential and are protected by appropriate security safeguards and practices to prevent unauthorised access, collection, use, disclosure, copying, modification disposal or similar risks, in accordance with the applicable laws.

9.  ACCURACY OF YOUR PERSONAL DATA

9.1 You should ensure that all personal data submitted to us is complete and accurate. Failure to do so may result in our inability to provide you with information or services you have requested.  

9.2 Well-Wellness Crown shall make a reasonable effort to ensure that personal data collected by or on behalf of the Well-Wellness Crown is accurate.

10.  CONSENT

10.1 By submitting your personal data to us, you agree and consent to the collection, use and disclosure of your personal data by us for some or all of the purposes mentioned herein.

10.2 If you provide us with personal information relating to a third party (e.g. information of your spouse, children, parents or relatives), you represent to us that you have obtained the consent of the third party to provide us with their personal data for the respective purposes.  

10.3 You may at any time withdraw any consent given in respect of the collection, use or disclosure of your personal data by giving prior notice in the form of a formal written request addressed to the Data Protection Officer. 

10.4 Please note that if you withdraw your consent to any or all use or disclosure of your Personal Data, depending on the nature of your request, we may no longer be in a position to continue to provide membership benefits and services to you. Such a withdrawal may therefore result in the termination of any membership that you may have with us.

11.  ACCESS AND CORRECTION OF YOUR PERSONAL DATA

11.1 You may request access to or make corrections to your Personal Data records, but Well-Wellness Crown has the right to charge a reasonable fee for processing your request. Please submit your request to our Data Protection Officer.
